In order for your report to be processed and investigated appropriately, it is

important that the report is as specific as possible. It is helpful if you consider the five

W-questions in your report: Who? What? When? How? Where?

 

Please make sure that your descriptions can also be understood by people outside. It is helpful if you are available forfurther questions.

 

The following information is helpful for processing:

• When and where did the violation happen?

• Persons suspected of being involved (surname, first name)

• Does anyone else have knowledge of the violation?

• If so, who (name, first name)?

• Describe the violation in your own words

• Attach documents if available Remain anonymous or not? If no, please provide first name, last name, email address.
